Event Recorder (monitoring and diagnosis of sporadic and symptomatic cardiac arrhythmias)
The service allows the remote monitoring of sporadic and symptomatic cardiac arrhythmias 24/24 through an Event Recorder.
Palpitations (perceived as irregular heart rhythm or feeling of missed heartbeat) is a symptom common to many patients and its presence can be difficult to document. The basic electrocardiogram and the recording of the dynamic electrocardiogram according to Holter are the most used methods to investigate this symptom; often they do not give encouraging results especially in patients in whom the symptom is “accessual”. In these cases, the aid of Telecardiology is a valid element in the diagnostic approach.
The Event Recorder is entrusted by the doctor to the user for a period of 7-15 days. The user, when the palpitations symptom occurs, applies the device to the chest, presses a button, records 30″ of heart rhythm (which is stored in the device) and sends it, transtelephone / telematic, to the HTN operations center where a cardiologist specialist provides the teleconsultation / report of the arrhythmic event.
The doctor of the facility can at any time access the user’s personal health file, in the “SmartTelemedicine Platform” of HTN, to:
- view the sent tracks

Blood pressure monitoring
The service allows the healthcare facility to entrust the user with a device for the detection of home blood pressure values (with bluetooth wireless transmission or NFC Near Field Communication).
All the pressure values detected, both at home and on the move, are automatically sent, through smartphones or tablets, to the user’s personal health file.
Their progressive memorization allows the HTN cardiologist the accurate definition of cardiovascular risk through the evaluation of pressor values, transient peaks, pressure load, variability, differential pressure and average heart rate.
The service offers the possibility to:
- carry out unlimited measurements at home and/or on the move of the pressure values;
- easily and permanently store all the pressure values detected;
- compare them with those present in the database;
- view value trends;
- periodically receive the report from the HTN cardiologist and take advantage of his 24-hour teleconsultation;
- access the printing of the report always and immediately;
- issue the user with a personal password for viewing the data.
The advantages are represented by the 200s of being able to:
- carry out checks in primary prevention for all those who are familiar with hypertensiveness, stress, obesity, kidney diseases etc …;
- carry out checks in secondary prevention (users with diabetes mellitus, cerebral stroke, dilated heart disease, etc…);
- evaluate the effects of antihypertensive therapy.
The service has two possible variants:
- the pressure detection device can be rented from the structure to its users who do not have a smartphone (in this case the user periodically returns to the clinic to download all the values recorded at home);
- it can also be activated through the use of blood pressure monitors already supplied to the clinics or to the user (in this case the pressure data are, from time to time, manually entered in the personal health dossier).
The 2013 Guidelines of the European Society of Hypertension underline that the blood pressure values detected at home have a prognostic significance more predictive of cardiovascular accidents than the values detected in the clinic.

The health facility therefore has a spirometer with which it can:
- record the spirometry track;

- request teleconsultation/reporting to the pulmonologist on duty.
The service is useful in order to:
- activate “screening” programs with respect to the population at risk (e.g. smokers/former smokers);

The health facility has a service that allows you to automatically detect and store, 24 hours a day, all the blood glucose values of its users.
The diabetic patient is entrusted with a USB / wireless transmission glucometer that (through PC / smartphone) sends in telematic mode on the web, to the personal health dossier, all the home measurements of glycemic values.
The health facility may, at any time:
- access the user’s personal health file;
- view the data sent by the device;
- print the report directly;
- request teleconsultation from the diabetologist on duty.
The service is useful in terms of users with diabetes mellitus who need to:
- constant tele-monitoring of hematochemical values (blood sugar, glycated hemoglobin, etc…);
- therapeutic adjustments (insulin, oral antidiabetics);
- specialized teleconsultations on their own co-morbidities (heart disease, retinopathy, arterial disease in the lower extremities, peripheral neuropathy, etc…).

“Intensive” clinic-instrumental telemonitoring
The “Intensive Telemonitoring” services are designed and implemented in favor of health facilities such as small hospitals, individual hospital divisions, nursing homes, RSA that do not have professional figures available 24 hours a day such as emergency doctors, cardiologists, anesthesiologists-resuscitators; the continuous and intensive health surveillance of critical patients is therefore, in full, delegated to HTN.
Instrumentation supplied to the facilities
In the structure are prepared beds with the following multi-parameter telemonitoring instruments that allow the remote control 24 hours a day of the vital parameters of the user by HTN’s specialist doctors.
Philips M3150 IntelliVue Monitoring Centre:
- monitors in real time and automatically the electrocardiographic, oximetric and blood pressure traces of the patient;
- allows the programming of specific alarms for each track;
- allows the simultaneous display of the tracks of 4 portable MP2 monitors and similar alarms (visual / sound) generated by the patient-monitor;
- provides for the export via the web of data to HTN, with the electronic sending of the traces of each alarm generated and its automatic archiving in the personal health dossier.
MP2 portable monitor connected to the IntelliVue monitoring panel:
- MP2 portable monitor connected to the IntelliVue monitoring panel. The monitor is able to display the values of blood pressure, oximetry, electrocardiographic waveforms and record a 12-lead track (with a 10-cable set) or continuous monitoring of the 3-lead track (with a 5-cable set). It is able to analyze the parameters of the track with respect to pre-established limits (always visible on the main screen), triggering an alarm in case of deviations.
Exceeding an alarm threshold is signaled by the monitor through the emission of an acoustic signal, of variable intensity depending on the severity.
In addition, a message appears that, for real alarms, flashes red in case of potentially dangerous situations, in yellow for simple conditions that exceed the preset limits of vital parameters.
Conversely, in case of technical alarms, the message on is fixed and blue (alarms from signal quality problems, malfunction or disconnections of the equipment).
How “Intensive Telemonitoring” works
Workflow:
- the doctor of the structure identifies the patient eligible for continuous intensive telemonitoring;
- access, through the authentication credentials (username and password) in the “SmartTelemedicine Platform” of HTN;
- subject to informed consent, enroll the patient (in the personal health dossier) by entering personal data and any other information useful for telemonitoring (anamnesis, therapy, events etc…);
- enters the patient’s personal data in the Monitoring Centre;
- applies electrodes/sensors (electrocardiograph, oximeter-blood pressure) to the patient;
- activate the device for continuous monitoring.
The Philips Intellivue control panel continuously and automatically analyzes the monitored traces, identifying any alarms for arrhythmias, beat/beat deviations in the measurement of QT, ST-T, blood pressure, oximetry.
The specialist on duty in HTN provides, in real time in the “SmartTelemedicine Platform” of HTN, the remote surveillance of the patient and the reporting of the signals received, making them available and printable to the department.
All alerted traces are stored in the patient’s personal health file.

Hospital protected discharges
The services designed and implemented to manage “protected hospital discharges” are addressed to health facilities (hospitals, individual hospital divisions, nursing homes) that intend to discharge in advance their patients suffering from chronic diseases (such as heart failure, diabetes, bronchopneumopathies, etc…).
For the realization of the protected discharge activities, HTN makes available to the health facility a software and services equipment structured as follows:
1. Telemedicine Station
The telematic station present in the health facility must provide at least:
- PC with Pentium processor 128Mb ram memory (or higher);
- CD ROM;
- 10/100 LAN card on board;
- Windows 2000 professional, Xp professional, Vista (or higher);
- Microsoft Word;
- Laser printer.
The connection to the HTN data center must be guaranteed in broadband mode.
The setting up of the remote telemedicine station involves the configuration of the software necessary for access by the health facility to the HTN SmartTelemedicine Platform.
In particular, the following are envisaged:
- “on site” health card software licenses for all enrolled patients;
- right to access and processing of proprietary sensitive health data.
For each workstation HTN guarantees the following set-up activities:
- configuration of the telematic station at the health facility by a specialized technician;
- roll-out of the remote connection with the HTN Service Center in Brescia;
- creation, at the HTN CED, of a hard disk partition for the configuration of the database dedicated to the activity of the health facility;
- system testing;
- training for medical/ nursing staff who will have to use the station.
2. Operational Management Flow of the service:
- the doctor in charge of the health facility reports to the HTN Service Center the name, subject to informed consent, and the relative follow-up plan of the patient in protected discharge;
- the Service Centre generates the personal health dossier;
- the patient is sent to the home, from time to time, with telemedicine equipment suitable for monitoring his clinical situation;
- the workflow includes scheduled contacts by the health facility to the patient and occasional contacts by the patient to the health facility whenever he feels symptoms or discomfort;
- for scheduled contacts, the Service Center, following the dates of the follow-up set by the health facility, provides to call the patient, to activate the possible reception of biological signals and to make them immediately available to the nurse tutor of the health facility;
- the nurse tutor of the health facility, in communication with the patient during the scheduled contact, updates the personal health dossier with the intervention plan and the agenda of the medical staff;
- at the end of the remote surveillance period HTN will notify the health manager and close the personal health file;
- HTN manages telemedicine equipment online;
- HTN periodically transmits to the health facility concerned the reporting of the activities;
- at any time, the personal health dossier is made available via browser or terminal server to users with the appropriate credentials (the computer system manages login, user profiling, authorization management and data back-up).
For all occasional contacts outside the time slots of activity of the health facilities (or night-holiday back-up or in case of non-response within five minutes) the Service Center intervenes in any case with its staff (nurse / specialist doctor) updating the personal health dossier and subsequently communicating to the head of the reference health unit the actions taken.
3. Organizational model:
The healthcare facility and patients can make an unlimited number of calls to the HTN Service Center.
The Call Center is active 24/365 days, with online response.
All HTN professional staff (medical specialists and nurses):
- is always on connected (VPN) in telework;
- has a consolidated experience in the specific specialist field at excellent healthcare facilities;
- attends periodic training and updating courses with respect to the main guidelines of the various pathologies.
All conversations that pass through the HTN Service Center in Brescia are recorded.
HTN guarantees full compliance with the legislation on sensitive personal data pursuant to Legislative Decree 193/2003.
These services allow Healthcare Facilities to activate a Telemedicine service at the chosen Hospital Division in a short time and “turnkey”, avoiding expensive structural investments.

The “Intensive Telemonitoring” services are designed and implemented in favor of health facilities such as small hospitals, individual hospital divisions, nursing homes, RSA that do not have professional figures available 24 hours a day such as emergency doctors, cardiologists, anesthesiologists-resuscitators; the continuous and intensive health surveillance of critical patients is therefore, in full, delegated to HTN.
Instrumentation supplied to the facilities
In the structure are prepared beds with the following multi-parameter telemonitoring instruments that allow the remote control 24 hours a day of the vital parameters of the user by HTN’s specialist doctors.
Philips M3150 IntelliVue Monitoring Centre:
- monitors in real time and automatically the electrocardiographic, oximetric and blood pressure traces of the patient;
- allows the programming of specific alarms for each track;
- allows the simultaneous display of the tracks of 4 portable MP2 monitors and similar alarms (visual / sound) generated by the patient-monitor;
- provides for the export via the web of data to HTN, with the electronic sending of the traces of each alarm generated and its automatic archiving in the personal health dossier.
MP2 portable monitor connected to the IntelliVue monitoring panel:
- MP2 portable monitor connected to the IntelliVue monitoring panel. The monitor is able to display the values of blood pressure, oximetry, electrocardiographic waveforms and record a 12-lead track (with a 10-cable set) or continuous monitoring of the 3-lead track (with a 5-cable set). It is able to analyze the parameters of the track with respect to pre-established limits (always visible on the main screen), triggering an alarm in case of deviations.
Exceeding an alarm threshold is signaled by the monitor through the emission of an acoustic signal, of variable intensity depending on the severity.
In addition, a message appears that, for real alarms, flashes red in case of potentially dangerous situations, in yellow for simple conditions that exceed the preset limits of vital parameters.
Conversely, in case of technical alarms, the message on is fixed and blue (alarms from signal quality problems, malfunction or disconnections of the equipment).
How “Intensive Telemonitoring” works
Workflow:
- the doctor of the structure identifies the patient eligible for continuous intensive telemonitoring;
- access, through the authentication credentials (username and password) in the “SmartTelemedicine Platform” of HTN;
- subject to informed consent, enroll the patient (in the personal health dossier) by entering personal data and any other information useful for telemonitoring (anamnesis, therapy, events etc…);
- enters the patient’s personal data in the Monitoring Centre;
- applies electrodes/sensors (electrocardiograph, oximeter-blood pressure) to the patient;
- activate the device for continuous monitoring.
The Philips Intellivue control panel continuously and automatically analyzes the monitored traces, identifying any alarms for arrhythmias, beat/beat deviations in the measurement of QT, ST-T, blood pressure, oximetry.
The specialist on duty in HTN provides, in real time in the “SmartTelemedicine Platform” of HTN, the remote surveillance of the patient and the reporting of the signals received, making them available and printable to the department.
All alerted traces are stored in the patient’s personal health file.

Hospital protected discharges
The services designed and implemented to manage “protected hospital discharges” are addressed to health facilities (hospitals, individual hospital divisions, nursing homes) that intend to discharge in advance their patients suffering from chronic diseases (such as heart failure, diabetes, bronchopneumopathies, etc…).
For the realization of the protected discharge activities, HTN makes available to the health facility a software and services equipment structured as follows:
1. Telemedicine Station
The telematic station present in the health facility must provide at least:
- PC with Pentium processor 128Mb ram memory (or higher);
- CD ROM;
- 10/100 LAN card on board;
- Windows 2000 professional, Xp professional, Vista (or higher);
- Microsoft Word;
- Laser printer.
The connection to the HTN data center must be guaranteed in broadband mode.
The setting up of the remote telemedicine station involves the configuration of the software necessary for access by the health facility to the HTN SmartTelemedicine Platform.
In particular, the following are envisaged:
- “on site” health card software licenses for all enrolled patients;
- right to access and processing of proprietary sensitive health data.
For each workstation HTN guarantees the following set-up activities:
- configuration of the telematic station at the health facility by a specialized technician;
- roll-out of the remote connection with the HTN Service Center in Brescia;
- creation, at the HTN CED, of a hard disk partition for the configuration of the database dedicated to the activity of the health facility;
- system testing;
- training for medical/ nursing staff who will have to use the station.
2. Operational Management Flow of the service:
- the doctor in charge of the health facility reports to the HTN Service Center the name, subject to informed consent, and the relative follow-up plan of the patient in protected discharge;
- the Service Centre generates the personal health dossier;
- the patient is sent to the home, from time to time, with telemedicine equipment suitable for monitoring his clinical situation;
- the workflow includes scheduled contacts by the health facility to the patient and occasional contacts by the patient to the health facility whenever he feels symptoms or discomfort;
- for scheduled contacts, the Service Center, following the dates of the follow-up set by the health facility, provides to call the patient, to activate the possible reception of biological signals and to make them immediately available to the nurse tutor of the health facility;
- the nurse tutor of the health facility, in communication with the patient during the scheduled contact, updates the personal health dossier with the intervention plan and the agenda of the medical staff;
- at the end of the remote surveillance period HTN will notify the health manager and close the personal health file;
- HTN manages telemedicine equipment online;
- HTN periodically transmits to the health facility concerned the reporting of the activities;
- at any time, the personal health dossier is made available via browser or terminal server to users with the appropriate credentials (the computer system manages login, user profiling, authorization management and data back-up).
For all occasional contacts outside the time slots of activity of the health facilities (or night-holiday back-up or in case of non-response within five minutes) the Service Center intervenes in any case with its staff (nurse / specialist doctor) updating the personal health dossier and subsequently communicating to the head of the reference health unit the actions taken.
3. Organizational model:
The healthcare facility and patients can make an unlimited number of calls to the HTN Service Center.
The Call Center is active 24/365 days, with online response.
All HTN professional staff (medical specialists and nurses):
- is always on connected (VPN) in telework;
- has a consolidated experience in the specific specialist field at excellent healthcare facilities;
- attends periodic training and updating courses with respect to the main guidelines of the various pathologies.
All conversations that pass through the HTN Service Center in Brescia are recorded.
HTN guarantees full compliance with the legislation on sensitive personal data pursuant to Legislative Decree 193/2003.
These services allow Healthcare Facilities to activate a Telemedicine service at the chosen Hospital Division in a short time and “turnkey”, avoiding expensive structural investments.
