The new role of the Pharmacist
(Clinical Pharmacist)

The second level services authorized by Ministerial Decree No. 153 of 2009 design a Pharmacy that can legitimately:
  • propose itself as a territorial health garrison of “public health services”
  • strengthen its central role in the area
  • provide customers with advice on behaviors and needs.
As part of the Pharmacy of Services, a collaboration agreement has been underway since 2014 with the University of Brescia for the realization of the project “Telemedicine in the primary and secondary prevention of cardiovascular diseases”.

    • Eletrocardiography

      The service provides the possibility of recording a 12-lead electrocardiogram in the Pharmacy.

      The service is useful:

      • in primary prevention for users at risk (diabetics, hypertensive, dyslipidemia etc ..) or for screening, in healthy subjects for sports practice, license, gym etc ..)
      • in secondary prevention after a cardiovascular event
      • for the onset of signs and / or symptoms of a cardiovascular nature
      • for controls related to the definition of cardiovascular risk
      • for controls related to the evaluation of the effects of cardiovascular therapies.

      The electrocardiography “in real time” is carried out through a device that records and sends the electrocardiographic trace to the user’s personal health file wirelessly..

      The Pharmacist can therefore, 24 hours a day at any time of the day and year:

      • make the recording of the electrocardiogram
      • send the electrocardiographic trace in telematic mode to the Service Center
      • request teleconsultation and immediate telereportation to the cardiologist on duty
      • provide directly to the printing of the report.

      The report and any related teleconsultation by the cardiologists of the HTN Virtual Hospital are provided within 10 minutes.

    • Diagnostics of cardiac arrhythmias with 24/48 Hours Holter Dynamic Electrocardiogram

      The Holter dynamic electrocardiography service allows the Pharmacist to make available to his users a device that records and sends, to the user’s personal health file, the entire sequence of the electrocardiographic trace recorded in 24/48 hours.

      The Pharmacist can, at any time, access the HTN “SmartTelemedicinePlatform” to:

      • view the sent tracks
      • print the report directly
      • request a teleconsultation from the cardiologist on duty.

      The service is useful for:

      • a diagnostic confirmation of the presence of arrhythmias
      • a more accurate definition of cardiovascular risk
      • an evaluation of the effects of antiarrhythmic therapy.

    • Event Recorder

      The service allows to document through an Event Recorder, 24 hours a day, sporadic, symptomatic cardiac arrhythmias.

      Palpitations (perceived as irregular rhythm or feeling of missed heartbeat) is a symptom common to many patients and its presence can be difficult to document. The basic electrocardiogram and the recording of the dynamic electrocardiogram according to Holter are the most used methods to investigate this symptom; often they do not give encouraging results especially in patients in whom the symptom is “accessual”. In these cases, the aid of Telecardiology is a valid element in the diagnostic approach.

      The Event Recorder is entrusted by the Pharmacist to the user for a period of 7-15 days. The user, when the palpitation symptom occurs, applies the device to the chest, presses a button and records 30″ of heart rhythm that is sent from the device, through the HTN APP, to the HTN operations center where a cardiologist specialist provides for the teleconsultation / telereportation of the arrhythmic event.

      The Pharmacist can access the “SmartTelemedicinePlatform” of HTN, to:

      • view the sent tracks
      • print the report directly
      • request a real-time teleconsultation with the cardiologist on duty.

    • Home monitoring of blood pressure

      The service provides for the sale / rental to the user of a device for the detection of home blood pressure values (with bluetooth wireless transmission).

      All the pressure values detected are automatically sent, through the HTN Telemedicine APP, to the user’s personal health file.

      Their progressive memorization allows the cardiologists of the HTN Virtual Hospital an accurate definition of cardiovascular risk through the evaluation of average blood pressure values, transient peaks, systo-diastolic load, variability, differential pressure and average heart rate.

      The service offers the possibility to:

      • take unlimited measurements at home and/or on the move
      • automatically store all the pressure values detected
      • compare them with those present in the database
      • view value trends
      • periodically receive the report from HTN cardiologists
      • take advantage of the teleconsultation of a specialist doctor, 24 hours a day
      • take advantage of the printing of the report, always and immediately
      • issue the user with a personal password for viewing the data by the attending physician.

      The advantages are represented by the 200s of being able to:

      • carry out checks in primary prevention for all those who are familiar with hypertensiveness, stress, obesity, kidney diseases etc ..
      • carry out checks in secondary prevention (users with diabetes mellitus, cerebral stroke, dilated heart disease, etc.)
      • evaluate the effects of antihypertensive therapy.

        N.B.

        The 2013 Guidelines of the European Society of Hypertension underline that the blood pressure values detected at home have a prognostic significance more predictive of cardiovascular accidents than the values detected in the clinic.

      • 24-hour outpatient monitoring of blood pressure

        The MAP/Ambulatory Blood Pressure Monitoring (or ABPM/Ambulatory Blood Pressure Monitoring) is a service that, through a device, records and sends to the user’s personal health file all the pressure values detected in the 24 hours.

        Their progressive memorization allows the cardiologists of the HTN Virtual Hospital an accurate definition of cardiovascular risk through the evaluation of average blood pressure values, transient peaks, systo-diastolic load, variability, differential pressure and average heart rate.

        The Pharmacist can at any time, with his password, access the HTN SmartTelemedicine Platform

        • view the data sent to the user’s personal health file

      • Telespirometry

        The service provides the possibility of recording a standard spirometry, with all the values related to lung capacity and the degree of airway parity.

        The Pharmacist has at his disposal a spirometer with which he can:

        • record the spirometry track
        • send it electronically, on the web, to the user’s personal health file
        • access the health dossier with the password to view the data sent
        • provide directly to the printing of the report
        • provide teleconsultation / telereportation in real time with the pulmonologist on duty.

        The service is useful in order to:

        • activate “screening” programs with respect to the population at risk (e.g., smokers/former smokers)
        • verify and confirm the certainty of the diagnosis of COPD (chronic obstructive pulmonary disease)
        • make a certain diagnosis of COPD flare-up whenever a patient stabilizes
        • define a diagnosis of certainty and have, through teleconsultation with the pulmonologist specialist, an appropriate therapy.

        The telereportation is made printable within 24 hours, while any related teleconsultation by the pulmonologists of the HTN Virtual Hospital is immediate.

      • Remote monitoring of oximetry

        The service includes the rental / sale of a wireless oximeter for extemporaneous or continuous remote monitoring of oximetry values.

        The user, through the HTN Telemedicine APP, transmits the oximetric data to the “SmartTelemedicine Platform” of HTN.

        The service allows the user to:

        • record the oximetric track in extemporaneous mode or continuously
        • send it automatically, on the web, to the user’s personal health file
        • access with your password to the personal health dossier to view the data sent
        • print the report directly
        • provide for teleconsultation / reporting with the pulmonologist on duty.

        The service is useful in order to:

        • activate “screening” programs with respect to the population at risk (e.g. smokers/former smokers)
        • verify and confirm the certainty of the diagnosis of COPD (chronic obstructive pulmonary disease)
        • make a certain diagnosis of COPD flare-up whenever a patient destabilizes
        • define a diagnosis of certainty and have, through teleconsultation with the pulmonologist specialist, an appropriate therapy.

      • Telediabetology

        The Pharmacist has at his disposal a service that allows its diabetic users to automatically detect and store, 24 hours a day, all blood glucose values.

        The user is sold or rented a USB / wireless transmission glucometer that (through the HTN Telemedicine APP) sends in telematic mode on the web, to the personal health dossier, all the home surveys of glycemic values.

        The Pharmacist and the user may, at any time:

        • log in with the password to the personal health dossier
        • view the data sent by the device
        • print the report directly
        • provide teleconsultation with the diabetologist on duty.

        The service is useful in terms of users with diabetes mellitus who need to::

        • a constant telemonitoring of hematochemical values (blood sugar, glycated hemoglobin etc …)
        • therapeutic adjustments (insulin, oral antidiabetics)
        • specialized teleconsultations on their own co-morbidities (heart disease, retinopathy, arterial disease in the lower extremities, peripheral neuropathy, etc…).

      • Telemonitoring and well-being

        Telemonitoring and wellness services are provided through the HTN Telemedicine APP and allow:

        • remote sensing of blood pressure and heart rate values
        • remote sensing of body weight and fat and lean mass indices
        • remote sensing of daily physical activity (pedometer, hours of sleep/wakefulness, calorie consumption).

        The services include the sale or rental of the related equipment, easy and practical to use, for the detection of the main physiological parameters.

        The user can, anywhere and at any time of the day and year:

        • carry out the detection of physiological parameters
        • send through the HTN Telemedicine APP to the Service Center the registered parameters
        • access your personal health file
        • view the data sent
        • print the report directly
        • request, if necessary, teleconsultation and immediate telereport to the specialist doctor of the HTN Virtual Hospital.

        These services are mainly addressed to users who have never had pathological events but who want to ensure themselves and their family members a control of health and well-being conditions (e.g. in primary prevention, during sports, leisure, etc.).

      HTN-Telemedicine App:

      APP HTN-Telemedicine (iOS or Android) collects in real time, in a personal health dossier, biological parameters such as blood pressure, blood sugar, oximetry, body weight, physical activity and many others.

      You can choose to:

      • manually enter the measurements carried out thanks to electromedical devices already in your possession;
      • use bluetooth devices integrated into iOS or Android platforms whose measurements are stored in your personal health file.

      It is possible to request, at the Pharmacies of the national network Federfarma-HTN:

      • medical teleconsultation
      • the report signed by the specialist, summarizing all the surveys stored with the interpretative comment of all the data both in their punctual value and in the trend of the same between them and over time
      • the credentials to access your health dossier for the insertion of clinical news, health services, laboratory investigations, diagnostic images, therapies and any other information you may need to have your medical history always available and immediately available in the APP
      • personalized programs of clinical-instrumental remote surveillance.

      Home and / or mobile remote surveillance is ensured 24h/24 by a medical-nursing team and promotes a safe and serene life both for the healthy user and for the user suffering from chronic diseases (cardiovascular, pneumological, diabetes mellitus, arterial hypertension etc …).

      HTN guarantees that the personal health dossier is protected according to current Italian and European regulations on privacy (HTN is GDPR compliant with EU regulation 679/2016).
